電療是什麼?
18世紀末義大利醫師
賈法尼( Luigi Galvani )因為電擊青蛙的肌肉,而開啟肌肉電生理的新頁。現代醫學中,止痛與促進神經肌肉功能,是 中風(腦傷) 者接受電療的主要目的。今天要討論的後者,是透過電刺激手腳上的神經和肌肉,造成肌肉收縮來產生動作。
動作的產生與學習
功能性的動作,都是經由中樞神經發出訊號到肌肉引發收縮才出現動作。這就像開車一樣,即使可以自動駕駛(肌肉自動收縮),但仍需要我們(腦)輸入指令,才能到達目的地 ( 做出動作 )。中風(腦傷),就像是開車的駕駛腳扭到或手骨折( 腦區受損 ),雖然會因為換腳踩油門或單手轉方向盤而覺得不順手,但多練習(
學習效應 )幾次後仍然會 改善 。
懶人復健——電療?
用電訓練肌肉,最一開始是幫助運動員 突破 傳統訓練的極限,所以
電療同時需也要做動作 才行。很多人總認為電療時手腳肌肉有在跳動,就是有『效果』,但其實都是 心理層面 居多。就如同前面強調的,有功能的動作,是需要經過
神經和肌肉系統 的 學習 。所以復健就是 動作練習 ,而 電療只是一種輔助!
電療無法防止萎縮
肌肉用越多就越緊實跟強壯,越少用就會鬆弛或萎縮。要維持肌肉的功能,就要保持神經與肌肉之間的互動,所以單靠電療,是
無法預防萎縮 的!最理想的做法,電刺激的同時做動作,而在電的輔助下,都能有更好的表現。
越強越好?
錯,而且錯得離譜!第一,很多中風(腦傷)的人感覺是有問題的,過度的電刺激只會
增加 電灼傷的風險。第二,電太強,會讓我們很 難 控制動作,不然用家裡插座的電,動作更大不是嗎?
總結
復健,就是學習如何轉動身體的齒輪,而電療則是一種輔助。所以效果要好,就必須要靠自己多練習、多做動作!
